If you are holding this book in your hands, please know that it is OUTDATED and OBSOLETE.  The printed version is a snapshot of how the book and some of its resources appeared at one point in time.  The online version is updated as needed.

Keeping up with all these areas is almost a full-time job in and of itself, and making sense of it all and keeping the end product from being internally contradictory is a never-ending task.
